Evidence by Condition
POTS · EmergingA single-dose crossover RCT (n=17) lowered standing heart rate and symptom burden over 4 hours. In a retrospective series of 203 treated patients, 43% (51% of those who tolerated it) reported improvement over about a year. No randomized trial has tested it alone over weeks or months.
ME/CFS · EmergingA single-dose RCT (n=45) preserved peak oxygen uptake and cardiac output on repeat invasive exercise testing. In a non-randomized conference abstract, 37 people who took it for about 2 years improved on exercise-test measures (oxygen uptake efficiency, pulmonary vascular capacitance), while 16 who did not take it did not.
Fibromyalgia · InconclusiveA 6-month RCT (n=165) found no improvement in pain or most symptoms, with modest gains in sleep and anxiety.

Mechanism of Action
Reversible acetylcholinesterase inhibitor; enhances ganglionic and vagal cholinergic transmission; attenuates orthostatic tachycardia; may support venous return and cardiac output
Evidence Type(s)
